Program Overview

A hospitalist is a physician who specializes in the care of a patient while admitted to a hospital for a medical condition or surgery. During a patient's stay, the HCMC hospitalist will contact the patient's primary or specialty care physician as needed to ensure the patient receives the best and most coordinated care possible. Upon the patient's discharge, hospitalist office personnel will mail or fax a copy of the patient's discharge summary and pertinent test results or reports to the patient's primary care physician.

The Hospitalist Program at HCMC accepts patient referrals from primary care physicians and non-physician providers in the community enabling medically-appropriate patients to bypass the emergency room and proceed directly to inpatient care.

To refer a patient, physicians and non-physician providers should call Hennepin Connect at (612) 873-4262 or 1-800-424-4262 and request to speak with the hospitalist on call.
