How to Obtain a Home Safety Assessment:

  • Ask your family doctor to arrange for a home assessment.
  • Contact your insurance company. Ask for your nurse navigator, MSHO case manager or social worker if you have one or call member services.
  • Call the Minnesota Visiting Nurses Association (MVNA) at (612) 617-4600 or visit www.mvna.org and request a home safety assessment.
  • If you are a veteran, you can speak with your primary care physician at your local VA medical center about a referral for a home safety assessment.
