The Center for Senior Care uses a case-management approach to coordinate community services and health care for individuals with multiple medical, social, or psychological problems. Geriatric specialists and nurse clinicians staff the center, providing geriatric assessment and consultation as well as ongoing primary care to seniors living independently and, through the Extended Care Program (see separate listing), to seniors living in long-term care facilities in the community.
