The Health Care for the Homeless Project is a federally
funded program administered by Hennepin County's Community Health Department.
Health care services for homeless individuals are provided by a mobile
interdisciplinary team of medical and social service professionals at
10 community-based shelters and drop-in centers. The team provides on-site
primary and preventive care, preliminary chemical and mental health assessments,
outreach, linkage and referral, eligibility assistance, and client advocacy.
Homeless individuals have 21 different clinical sessions available to
them weekly.
