Welcome to Best of Hennepin!

We look forward to having you join us at the Best of Hennepin Continuing Medical Education Event hosted at the Holiday Inn Metrodome in downtown Minneapolis, October 20-22, 2011. This conference will focus on the practice of patient- and family-centered care to engage patients, specialists and care teams in the successful management of chronic disease.  It will feature patient panels, specialists, and primary care providers discussing the challenges of this new approach in the care of people with chronic diseases such as heart failure, diabetes, cancer, asthma, renal disease, and orthopaedic problems. It will also address some of the specific challenges faced by homeless patients or those living in remote areas.

Physicians - Accreditation – Hennepin County Medical Center is accredited by the Minnesota Medical Association (MMA) to provide continuing medical education for physicians.

  • Designation – Hennepin County Medical Center designates this live activity for a maximum of 15.00 AMA PRA Category 1 Credit™.  Physicians should only claim credit commensurate with the extent of their participation in the activity.
  • Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.

Nurses – This activity is designed to meet the Minnesota Board of Nursing’s continuing medical education requirements.  Nurses are responsible for determining whether this activity meets the requirements for acceptable continuing education credit.

Pharmacy Credits are pending.

Other  Health Care Providers – Other health care providers may use course materials to submit to their professional organizations for individual credit.

Provider Educational Needs Assessment:  We have planned this activity to enhance the ways in which health care providers identify, manage and approach processes they have worked with for many years. The main goal of this conference is for providers to take on a leadership role of improving patient safety, care and outcomes by focusing on patient and family centered care through the construct of all they do.

Educational Learning Objectives:
Following this conference, attendees will be able to:

  1. Improve and provide leadership for how patients, families, and providers interact with each other in managing chronic diseases.
  2. Recognize how the Medical Home will facilitate the interactions described in #1.
  3. Analyze how improving health care for the homeless and recent immigrants to the U.S. is an issue for all providers, and identify how such improvements reduce chronic disease in these patient populations.
  4. Demonstrate improved management strategies for patient care in the areas of: Diabetes, Obesity, Hypertension, Colon Cancer, Asthma, Renal Disease, Chest Pain, Gestational Diabetes, & Renal Disease.
  5. Improve quality of care for patients returning to their primary care provider following trauma or multi system disease care from a tertiary care center.

Friday Evening Activities and Events

Tour of Whittier Clinic: October 21 7-8pm

All attendees of Best of Hennepin are invited to tour Whittier Clinic, located at 2810 Nicollet Avenue South in Minneapolis, on October 21 from 7pm-8pm. Whittier Clinic, opened in October of 2010 to great fanfare and is home to HCMC’s family medicine residency program. In addition to primary care services, the clinic offers a wealth of health care specialties. Construction of the building earned LEED certification for its sustainable design and features. The tour is free to all attendees. RSVP to amy.carlson@hcmed.org.

We also encourage you to join us right across the street at 8pm for the Hennepin Homecoming party.


Dance the night away at Hennepin Homecoming, a party supporting the education mission of HCMC.

Join staff, alumni and attendees of Best of Hennepin at a dance party celebrating and supporting education and training programs at HCMC. Groove to a play list created by you! Enjoy appetizers, cash bar and the unique atmosphere of Old Arizona. A collection will be taken that evening to help support a new Innovation in Medical Education Fund.
