The Hennepin Diabetes Education Program

The Hennepin Diabetes Education Program offers comprehensive education and training
designed to help you to:

• Be empowered to control diabetes
• Be and feel healthy
• Improve your quality of life.

If you have diabetes and have not had the opportunity to participate in diabetes education,
or would like follow-up education, please contact your primary care provider, or your
diabetes care provider, to request a referral.
