Trauma Referral Services

HCMC receives more then 1000 trauma referral patients a year. The majority of these patients are first assessed and stabilized at hospitals throughout the upper Midwest region. They are then transferred
to HCMC via helicopter or ground transportation.

HCMC has established an extensive referral system in order to provide timely communication with referring facilities.

• Emergency physician/nurse call back after initial assessment
  
• Attending trauma/surgery staff call back within 48 hours of admission
  
• Timely follow-up communication
  
• Dictated discharge summaries sent to referring physicians upon patient discharge

HCMC’s Trauma Referral Follow-up Service coordinator is responsible for the collection of information on referred trauma patients to HCMC. The main objective of this service to disseminate information to the referring and/or primary physician of the patient’s status and, upon discharge, a review of the care the patient received during their hospital stay.
