Center for Senior Care Extended Care Physicians within the Division of Geriatrics provide primary care and assessment/ consultative services to the elderly in a variety of settings. Staff geriatricians and nurse practitioners provide inpatient, outpatient, and nursing facility care. Patients who are likely to benefit from geriatric medicine evaluation include those with memory or mood problems, incontinence, gait instability, falls, chronic illness, and complex drug regimens, as well as those for whom nursing home placement is being considered. Services include: HCMC Senior Care Clinic Primary care on the campus of Hennepin County Medical Center, Level 1 Trauma Center health facility in downtown Minneapolis Click here for a printable Senior Care Clinic brochure Extended Care Services The primary purpose of Extended Care is to provide primary care services to patients in 20 nursing facilities in the Minneapolis and surrounding area. Service is provided by a physician and nurse practitioner team who see the patient in the nursing facility. This team is supported by nurses and other staff in the Extended Care office at Hennepin County Medical Center. Staff accepts referrals of patients from HCMC inpatient units. The team also accepts referrals from sources outside Hennepin. Extended Care staff can be a resource for complex discharges from HCMC and other medical facilities.
