and Overnight Passes
A home pass is highly recommended close to discharge. This will allow the family and rehabilitation team the chance to see how the patient functions in the home and community. A pass may also help with discharge planning. Sometimes unforeseen problems occur during a pass. A pass questionaire will be given to patients and families with questions pertinent to how that pass went. This gives the family, patient, therapists and physician an opportunity to correct any problems encountered on the pass prior to discharge home. Community and home passes also help motivate the patient for the transition home and for increased independence.
