The primary goal of our rehabilitation program is to return the patient home to their family and community. The Knapp social worker will help coordinate the services a patient will need following discharge. In order to create an appropriate discharge plan, the social worker will need to gather a patient history which includes:
- Social history
- Financial and insurance information
- Family support
- Current living situation
- Medical history
- Work history
- Alcohol/chemical issues
- Transporation needs
This psychosocial evaluation is completed to determine the need for social service interventions. Patients are not always able to give accurate information following a brain injury. The social worker will rely on a close family member to provide the information needed to plan for a patient discharge. If a patient is not able to care for himself on his own and has no family to provide consistent care, the social worker will review options with the patient and family.
Social Services personnel help patients navigate the multiple hospital and community systems they encounter while encouraging patient autonomy. Staff are committed to justice and equality for all patients and to enchancing patients’ lives within context of health and healing.
