Follow-up Survey

At three to four months after discharge, patients will receive a confidential over-the-phone survey. This survey is important for several reasons. We want to determine if patients are satisfied with the Knapp Rehabilitation Center. We also want to determine how the patient is functioning after discharge. At the time the survey is given the majority of patients are improving or stabilizing. The follow-up survey information is meaningful to the Knapp Rehabilitation Center so we may continually improve our services for each patient.
