Transition
Patients that
are either in school or in the process of attending school may be
in need of assistance following hospitalization. The Knapp Rehabilitation
Center team is qualified to help with issues related to returning
to school. We work closely with patients still in high school and
help coordinate a plan for return to school. School personnel are
invited to family conference meetings to help develop plans for returning
to school. There is a coordinated effort to create a plan to meet
the needs of the individual student. A plan might involve an IEP (Individual
Education Plan), which can help tailor a program for individuals that
experience a change in their abilities following brain injury.
Vocational
Transition
Returning
to work is a primary goal of rehabilitation. When patients are discharged
from the rehabilitation program they often continue as outpatients.
Outpatient services focus partly on issues concerning return to work.
In some cases patients may be referred to vocational services such
as the Minnesota Department of Jobs and Training, which is part of
the Division of Rehabilitation Services. Most of our patients eventually
return to work following discharge. If a medical condition exists
that limits a person's mobility, weight-bearing, vision, memory, thinking,
or other job critical skills, the rehabilitation team will work to
improve the skills necessary for returning to work.
