PGY-2 Pharmacy Residency Programs
Residency Director: Tracy Anderson-Haag, Pharm.D, BCPS Purpose The purpose of this 12-month PGY-2 Specialty Residency in Solid Organ Transplantation at Hennepin County Medical Center is to develop the resident’s expertise, knowledge and skills in the pharmacotherapeutic management of internal medicine issues in patients with a solid organ transplant. The resident will learn to conceptualize, integrate and transform accumulated experiences and knowledge into new and improved pharmacy services. After completion of this PGY-2 Solid Organ Transplant residency program the graduate will be a highly capable clinical practitioner with advanced skills in transplant related issues who is able to pursue clinical, academic, research or industry-based career pathways.
Activities involving direct patient interaction in an inpatient and ambulatory setting, physician consultation, teaching and clinical research are major components of this specialized solid organ transplantation residency. The resident will be expected to substantially contribute to the advancement of the department’s mission and goals. Preceptors will provide the training, guidance and instruction for the resident while participating in hospital and clinic activities. Learning experiences (lectures/seminars), evaluations and interdepartmental activities are included in the residency program.

About Hennepin County Medical Center Transplant Program Hennepin County Medical Center (HCMC) is a 471-bed Level 1 Trauma Center located in downtown Minneapolis. Our transplant program at HCMC has played a long-standing and vital role in the treatment of chronic kidney disease. Established in 1963, HCMC was the first transplant center in the Upper Midwest and one of the first successful programs in the country. Clinical pharmacists have been an active part of this treatment team since 1985. We currently perform approximately 90 kidney transplants per year. At HCMC, we have performed more than 2475 kidney transplants, with an increasing percentage involving living donors. Our experience, along with our use of new therapies, diagnostic tools, and surgical procedures, allow us to give our transplant patients and living donors the best possible care. HCMC's Renal Transplant Program was selected as one of three "top performers" in the Transplant Services 2003 Benchmarking Project conducted by University Health System Consortium (UHC**). Our transplant program is a collaborative effort among healthcare professionals who specialize in the re-transplant, operative, and long-term follow-up care of ESRD patients, in partnership with their primary physician or nephrologist.

This transplant project examined many clinical and administrative performance measures submitted by 46 transplant centers, including many of the best-known centers in the country. |
