What you will need:
The process:
1. Now, get an application from the Minnesota Board of Medical Practice by calling them at 612-617-2130. Start filling it out. Note: you will be asked what type of privileges you have, then answer is "none" - even as residents, we are not considered to have practice priviliges.
2. Get self-query info from the National Practitioner Data Bank/Health Integrity and Protection Data Bank (NPDB/HIPDB). Download the form from the website www.npdb-hipdb.com under self query. Complete the form and send it back as instructed. You will get an envelope back in the mail - DO NOT OPEN IT - just forward it unopened to the Board with the rest of your application.
3. Submit your USMLE exam scores. Go to www.fsmb.org and click "transcript request". This'll set you back $50.
Applying for your DEA license
BE WARNED!! After careful discussion with someone in the Chicago DEA office - a DEA number issued under the "exempt" status - free, because you work in a county facility can only be used at HCMC. It cannot be used when you moonlight, nor can it be used at any outside facility once you graduate. The only way you can keep this number and use it later is if you renew it at the 3 year point.
