Diabetes

FMC participates in the Staged Diabetes Management program (SDM), which advocates a standard and proven care process for diabetics. We believe care is optimized when providers treat patients in a consistent, evidence-based manner. Nurse educators and dietitians are available in the clinic as well to assist our patients in learning about and living with their diabetes.

Obstetrics

Continuity of care for the pregnant patient is an important aspect of the health care services we provide to our community. The same physician that cares for them during their pregnancy will also attend the birth, care for the newborn in the nursery, and provide well child care in the clinic afterwards. This comprehensive approach embodies the philosophy of family medicine at its best
and is integral to all aspects of our residency program.

Asthma

As an active member of the ICSI (Institute for Clinical Systems Improvement), HCMC is committed
to evaluating and improving the patient care experience. We use the evidence-based processes developed by national consensus organizations to bring our patients the highest quality care available.

Residents are taught and expected to use up-to-date guidelines, education and action plans to achieve optimal patient improvement. Faculty are knowledgeable in the processes as well and ongoing education assure the latest information is available. The new Electronic Heath Record also supports these innovative processes.

Integrated Medicine in Residency (IMR)

Hennepin joined seven other residency programs from across the U.S. in 2008 in an IMR pilot
project. (It is not a fellowship.) This project complements what we already teach in our
