The Positive Care Center services include:

Primary Health Care – Our health care providers are extensively trained in providing quality care to our diverse patient population. Several providers co-manage multiple chronic illnesses for their patient.  We also provide other specialty services as needed, either in the clinic or by referral.

HIV Care to Uninsured/Underinsured Patients – The Positive Care Center receives Ryan White funding that enables us to provide high-quality care to persons living with HIV regardless of their ability to pay.

Social Service Support – Social workers and case managers are on-site to provide assistance for numerous issues that may arise within the course of a patient’s care. Our social workers can assist with housing needs, transportation assistance, applying for financial assistance, and clarifying insurance questions.

HIV Testing – The Positive Care Center is able to provide pre-test counseling and rapid-testing to partners of our patients at no charge. This testing is made possible through a grant from the Minnesota Department of Health.

Case Management Services – Case management is intended to support the client by identifying and addressing needs through an individualized plan of care. Case management services are available on-site to respond to the broad range of physical, emotional, and social needs that individuals living with HIV may experience.

Research Opportunities – The Positive Care Center Research Program provides patients with opportunities to enroll in a variety of clinical trials and research protocols. Clinical trials often allow participants’ access to the newest therapies that are available only in a research setting.

Health Education and Risk Reduction Counseling – The Positive Care Center has a Health Education/Risk Reduction Counselor available to meet with patients and their partners, families and /or caregivers to provide more information on living healthy with HIV and reducing the risk of transmission of HIV and other sexually transmitted diseases.

Medication Adherence Supplies and Education – One of our goals at the Positive Care Centers is to help make taking medications as easy as possible. Pharmacists are on staff to meet with patients to discuss their medication taking behaviors and provide additional information regarding side effects and adherence. In addition, we offer practical interventions  that help patients remember to take their medications.

Psychotherapy Services – People living with HIV often experience a variety of emotions such as: anger, sadness, depression, guilt, anxiety, and fear. Many people also struggle to cope with sexuality identity, drug and alcohol use, and acceptance by friends and family. The Positive Care Center has a licensed psychotherapist and a certified psychiatric nurse specialist who can meet with patients to talk about how to decrease stress and improve their mental health.

Nutritional Assessments – A nutritionist assists our patients in creating a diet that helps the body to maintain good health, while also setting realistic nutritional goals.

Interpreters – Hennepin County Medical Center Provides interpreters for patients who wish to communicate in another language. There are a wide variety of languages interpreted including: Spanish, Oromo, Russian, Amharic, French and Somali.

Transportation Services – Patients may be eligible to receive assistance with transportation to clinic appointments.
